    Chloroquine mitophagy

Three forms of autophagy are commonly described: macroautophagy, microautophagy, and chaperone-mediated autophagy (CMA). In macroautophagy, expendable cytoplasmic constituents are targeted and isolated from the rest of the cell within a double-membraned vesicle known as an autophagosome, which, in time, fuses with an available lysosome, bringing its specialty process of waste management and disposal; and eventually the contents of the vesicle (now called an autolysosome) are degraded and recycled.

The antimalarial drug chloroquine CQ disrupts autophagy by inhibiting the acidification of the lysosomes that fuse with the autophagosomes, thereby preventing the degradation of metabolic stress products and inducing apoptosis. Chloroquine-mediated inhibition of autophagy has been demonstrated in melanoma. Hannah R. S. Fay, Kaitlyn M. Dykstra, Matthew Johnson, Tara L. Cronin, Linda Lutgen-Dunckley, Brandon L. Martens, Joseph R. Moberg, Monica L. Guzman, Eunice S. Wang; Mitophagy Plays a Key Role in the Anti-Leukemic Activity of Autophagy Inhibitors Under Hypoxia in Acute Myeloid Leukemia. Oct 01, 2018 Chloroquine phosphate tablets should not be used in these conditions unless the benefit to the patient outweighs the potential risks. Usage in Pregnancy Usage of Chloroquine during pregnancy should be avoided except in the prophylaxis or treatment of malaria when the benefit outweighs the potential risk to the fetus.

In the extreme case of starvation, the breakdown of cellular components promotes cellular survival by maintaining cellular energy levels. In disease, autophagy has been seen as an adaptive response to stress, promoting survival of the cell; but in other cases it appears to promote cell death and morbidity.

  4. A severe eye problem has happened with chloroquine. This may lead to lasting eyesight problems. The risk may be higher if you have some types of eye or kidney problems. The risk may also be higher with some doses of chloroquine, if you use chloroquine for longer than 5 years, or if you take certain other drugs like tamoxifen.

Figure 1. Autophagy Mechanisms and Signaling Pathways. Autophagy including macroautophagy, mitophagy and microautophagy is an intracellular degradation system that delivers cytoplasmic constituents to the lysosome often mediated by LC3. Chloroquine is a lysosomotropic agent that prevents endosomal acidification 1. It accumulates inside the acidic parts of the cell, including endosomes and lysosomes. This accumulation leads to inhibition of lysosomal enzymes that require an acidic pH, and prevents fusion of endosomes and lysosomes. Plaquenil and nightmares I was on Plaquenil from 2013 to March 2017. I never had any side-effects. My Rheumatologist took me off the drug when I started Cellcept. I got so many infections sinusitis especially on Cellcept, that I had to stop taking it. So in November 2017, I was started on Plaquenil again because my overall health had dramatically deteriorated.
